Job Details

  • Position: Women’s Health Social Worker / Coach (Registered Social Worker, member of the OCSWSSW
  • Location: Primarily work from home and located near our downtown office locations (Greater Toronto Area or Greater Montreal). Occasional in-person days are required. This may change for certain weeks—company events, or training, while offering flexibility for remote work.
  • Employment Type: Full-Time
  • Reports to: Coach Team Leader (for administrative, operational, and performance management); with clinical escalation pathways to the Director of Patient Care

As a Women's Health Social Worker and Coach, you will be at the heart of supporting women through every stage, from peri- to post-menopause, and beyond. You’ll offer virtual care, providing informed psychosocial and behavioral advice backed by our medical team, while delivering compassionate education and creating personalized care plans tailored to each woman's unique lifestyle needs. Collaborating with nurse practitioners, doctors, and nurses, and coaches you’ll implement practices based on the latest scientific data. What You’ll Do:

  • Officially oversee the psychosocial and behavioral care of all clients in collaboration with the nurse.
  • Conduct virtual behavioral assessments and evaluate lifestyle barriers reported by clients.
  • Develop and adjust personalized lifestyle care plans, including stress, sleep, and macro-lifestyle recommendations.
  • Intervene when flagged by the care team or clients, addressing:
  • Worsening or new mental health or emotional symptoms that require clinical social work expertise.
  • Changes in client satisfaction, motivation, or behavioral health outcomes.
  • Any psychosocial barriers to care that arise (e.g., burnout, stress, food insecurity).
  • Escalate any situations that are outside of your expertise (such as medical symptoms or specialized dietary needs) to the Registered Nurse (RN), Nurse Practitioner (NP), or Doctor (MD).
  • Conduct regular behavioral follow-ups to support clients in adhering to their lifestyle goals, managing stress levels, and navigating the emotional impacts of perimenopause and menopause.
  • Collaborate closely with the medical team to ensure that psychological support aligns perfectly with prescribed medical interventions (like Menopause Hormone Therapy).
  • Support and provide education to the team, ensuring they understand behavioral barriers, cognitive strategies, and psychosocial care interventions.
  • Keep the Electronic Medical Record (EMR) updated with all client information, ensuring thorough, accurate, and audit-ready documentation that ties lifestyle goals back to holistic mental well-being.
  • Ensure compliance with professional standards, clinical guidelines, and evidence-based practices.
  • Act as the behavioral and emotional point of contact for escalated client needs or questions.
  • Provide empathetic virtual care and behavioral coaching on menopause, stress reduction, sleep hygiene, nutrition habits, fitness behaviors, and holistic wellness for mid-life women.
  • Empower and educate women, guiding them towards healthier, more fulfilling lives.
  • Contribute to the creation of impactful resources designed to support our clients effectively.

In your quarterly reviews, you will be measured on and receive feedback on:
  • The quality of the behavioral and therapeutic advice you provide.
  • Your clients’ health and lifestyle improvements.
  • Their engagement with Coral.

What We’re Looking For:

  • English fluency required
  • French fluency is an asset
  • ON license required (OCSWSSW), with additional QC license (OTSTCFQ) preferred.
  • Clinical and coaching expertise: You are a registered social worker with experience in women’s mental health, reproductive transitions, and holistic wellness, with a focus on supporting women through peri- to post-menopause.
  • Certification or training: Strong knowledge of sleep hygiene frameworks (e.g., CBT-I), stress-reduction modalities, and the behavioral aspects of nutrition and exercise. Mastery of motivational interviewing and cognitive behavioral techniques.
  • Tech-savvy and digitally agile: You have a high level of comfort with technology and thrive using a modern tech stack. You can seamlessly navigate and switch between different platforms (e.g., electronic medical records, secure messaging apps, video conferencing tools, and internal communication channels) while maintaining a professional and smooth client experience.
  • Comfort with remote work: You are comfortable working in a virtual setting and using online medical consultation platforms to connect with clients and manage care.
  • Attention to detail: You are thorough in client behavioral assessments, motivation tracking, and clinical documentation, ensuring every aspect of psychosocial care is accurately recorded in the EMR.
  • Strong communicator: You can explain behavioral strategies, lifestyle modifications, and emotional wellness tools to both clients and the medical team clearly, making sure everyone is on the same page.
  • Collaborative team player: You are comfortable working closely with Nurses, providing behavioral oversight and integrating your care seamlessly with medical interventions.
  • Problem solver: You can make critical decisions when client motivation dips or emotional symptoms change, ensuring the client’s holistic health is always prioritized.
  • Client-centered: You are dedicated to improving client outcomes, especially with symptom relief, habit sustainable formation, holistic approaches, and long-term wellness improvements.
  • Adaptable: You can manage the fast-paced nature of virtual healthcare while maintaining high standards of clinical and behavioral care.
